Oral Surgery - Extractions

Why Does My Tooth Require Oral Surgery?

Sometimes when teeth are damaged beyond repair, surgery is simply the best option for a patient. Wisdom teeth can become impacted and when that happens, surgery is indicated so the situation doesn't have a chance to worsen. Extraction may be necessary when crowding, extensive decay or trauma are involved, and oral surgery provides the best avenue for safely removing the tooth and causing minimal disruption to the surrounding area.

Before the tooth is pulled, you will be given local anesthesia. Local anesthesia will remove any sharp pain but you will still feel pressure during the procedure. Once the anesthesia wears off, it is normal to expect some discomfort or pain, and there may also be some swelling and a small amount of bleeding. If these side effects persist longer than four hours after surgery, you should contact your dentist for additional treatment.

Why Can't Your Tooth Be Saved?

There are some situations where infection and tooth damage are so prevalent and widespread that a tooth simply cannot be saved. When the pulp tissue is infected and inflamed, it is often beyond the state where it can be recovered, and extraction then becomes the only option. Antibiotics will not be able to reach the problem and clear it up. Severely infected teeth are often so badly affected that they become dangerous to nearby teeth, and cannot be saved.

Types of Extractions

Simple vs. Surgical

A simple extraction is one where the tooth to be extracted is visible and above the gum line. When the tooth is somehow trapped in the jaw or impacted, it may be necessary to remove some bone or gum tissue in order to even access the tooth. In these cases, the tooth will need to be surgically removed. It is generally necessary to close the wound to the gum line with stitches afterwards.

Keep the gauze in place for two hours after surgery, and avoid doing any strenuous exercise. Don't drink through a straw for three days, and don't spit for 24 hours following surgery. Eat only soft foods and drink liquids for the first 24 hours after the extraction. Avoid hot and carbonated drinks. Apply ice packs as needed to alleviate pain and swelling. It is important to avoid smoking because this may lead to a very painful dry socket.
